The Recovery Navigator facilitates the Getting to Know You process for each participant served in the Restart program in partnership with a Peer Recovery Coach. The Recovery Navigator facilitates the creation of Recovery Solutions identified by the participant within the 9 Dimensions of Wellness that respond to their self reported immediate needs. The Recovery Navigator utilizes Recovery Coaching and the Recovery Pathways to assist the participant in achieving their identified solutions. The Recovery Navigator serves as the liaison between the established clinical home, as applicable, with the participant and assists with locating community resources to enhance options for achieving identified Recovery Solutions. The Recovery Navigator acts as an advocate for the needs of every participant and works in collaboration with the Restart leadership team to support the day-to-day activities of the program’s Recovery environment.
Essential Job Functions
Participates in Welcome process with Recovery Coach and participant within 24 hours of referral receipt. Ensures Welcome Packet, including consents, is understood and completed.
Utilizes Recovery Coaching to facilitate Getting to Know You processes with the participant. Capturing information in the Recovery Journey, Recovery Innovations Electronic Recovery Record.
Partners with participant to ensure collaboration with current supporters identified i.e. family, friends, clinical home, housing supports, to obtain collateral information and ongoing participation in identified recovery journey.
Creates Recovery Solutions with the participant identifying immediate needs within the 9 Dimensions of Wellness.
Identifies and educates participant on community resources to achieve success with their Recovery Solutions.
Acts as a liaison with the EEU (DSAMH Eligibility and Enrollment Unit) to provide coordination of care and updates regarding identified progress toward identified Recovery Solutions.
In partnership with the participant, serves as an advocate with psychiatric providers and other service providers in order to achieve success toward identified Recovery Solutions within the 9 Dimensions of Wellness.
Serves as the mental health professional for participants until they are enrolled in services at another community agency, when applicable.
Provide support with identifying and pursuing eligible benefits including, but not limited to housing assistance, social security benefits, Medicaid and/or Medicare benefits, prescription benefits when applicable.
Participates in on-call after hours rotation as assigned.
Drivers personal or agency vehicle to conduct agency business.
Other duties as assigned.
Education & Experience
Masters Degree in Psychology, Social Work or related field from an accredited college or university.
Two (2) years experience providing psychosocial/mental health services and/or crisis intervention services in a mental health setting
Delaware Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor of Mental Health (LPCMH), Certified Alcohol and Drug Counselor (CADC) or Certified Co-Occurring Disorders Professional (CCDP) preferred.
