The STAR Case Manager, Institute for Prevention, promotes the STAR individual's recover to their fullest potential. The Case Manager provides a comprehensive case management assessment that addresses life domains such as housing, finances, transportation, legal services, vocational, employment, health care, and family needs and strengths. Additional responsibilities include but are not limited to: Developing an Integrated Family Case Plan that is person centered and includes strategies for recovery; Linkage to appropriate care and resources in the community, including ones that address specialized needs; Coordinate needed interventions and services provided by multiple agencies; and Provide training to clients on the recognition of relapse triggers and strategies to deal with avoiding a relapse.
*Bachelor's degree required / Master's degree preferred in behavioral health profession
*CADC or LCADC required
*Must possess the knowledge, skills and experience necessary to completely perform case management and have at least three (3) years demonstrated experience working with individuals with substance use disorder
